Consequence: whenever the ip of the active mgr changed, all iscsi daemons would be reconfigured with the new trusted ip list

Fix: if the active mgr ip is already in the trusted ip list, the list is left alone

Result: If users put the ip of each mgr host in the trusted ip list, cephadm will no longer reconfigure the iscsi daemons after a mgr failover.

.`cephadm` does not reconfigure the iSCSI daemons after a manager failover

Previously, `cephadm` would always add the active manager IP to the end of the trusted IP list, regardless of whether the IP was already in the list. Consequently, whenever the IP of the active manager changed, the iSCSI daemons would be reconfigured with the new IP list.

With this fix, if the active manager is up already in the trusted IP list, the list is left alone and `cephadm` does not reconfigure the iSCSI daemons after a manager failover.
