How reproducible:
Always

Steps to Reproduce:
1. Install ldap with free_ipa
2. Log in to UI and leave the UI until time out
3. After time out, try logging back in

Actual results:
UI shows an error (as shown below): (Screenshot attached)
ERROR: duplicate key value violates unique constraint "index_roles_users_on_user_id_and_role_id" (PGError)

Correction: I am able to log in.  But I see the UI error (and error in the logs) for each action.

So far I faced this in the following ldap modes:
1. free_ipa
2. Active Directory
