Steps to Reproduce: 
Have entered into a plan.


1.Click "Reviewing Cases" label.
2.Choose one case and click the Test Case Summary.
3.Input some comment.
4.Click "Add comment" button.



Actual results: 
Sometimes no comments can be added. see attachment for error that catch by firebug. This issue can not be repruduced always. after try again and again maybe can be added

Expected results:
After step 4, comments have been added.

verify this bug on v3.8.8-1 on devel server.--->Pass

verify steps:
1.Enter a plan detail page, enter reviewing case tab, can refer the follow link.
https://tcms-devel.app.eng.nay.redhat.com/plan/10121/qiyang-test-plan-a#reviewcases
2.Expand the test case
3.Input some comment. 
4.Click "Add comment" button.

Actual results:
After step 4, comments have been added.

This problem happened again on stage and production server now. 
After click 'add comment' button, internal error will response upon the ajax request of '/post/'.
After try several times,I found it can not reproduce 100%.
It will occur after service is online for a long time.
Please verify this issue before the latest release.

verify on devel server with 3.8.12 and this issue still exists.
We can add the same comments only via copying instead of inputting.
