The following changes must be made:

* Remove step titles.
* Break down large steps into their component substeps.
* Convert large blocks of text into 'Note' boxes where necessary.
* Ensure all current admonitions are correctly classified (should any 'Note' boxes actually be 'Important' boxes?).
* Add one or two lines to the opening 'Summary' formal para block to clarify why a Squid Proxy is and why a user would consider configuring one.
* Remove any unnecessary explanatory text from steps.
