after_request_task_create was called twice during service provision. As a result, when provisioning a VM through a service and using the functionality of the default naming method, server names were incremented by 2. An extra call to after_request_task_create was removed as create_request_task already calls after_request_task_create. This has now fixed the issue; VMs provisioned through services increment by 1 (one).

Verified machines are named in single step increments -> test001 test002 test003 in 5.3.4.2.20150415125246
