The service is running it appears:

I went back through my console history and noticed the fix_auth --db command didn't work from the migration document, so that might be part of the issue.:

fix_auth --db has been available since 5.4.0.0-beta2

Either they are running an unsupported version of 5.4 or they did not upgrade properly.

Do we know what version of the product they are running?
Do we know what steps (or document) they followed to upgrade?

They just updated from 5.2.x to 5.4.0.5. For some odd reason that is not there even though he has the supported version.
