is this consistent? e.g does it always behave that ways or only in certiain combinations? reason I'm asking is that it looks like there was some sort of a failure, for example, the mac field should not be visible at all when provisioning a vm.

can you please, try again, this time ensuring you are not selecting the compute resource (deploy on) before other operations such as selecting a hostgroup, puppet environment etc is finished (e.g. wait for the spinner).

another information that is missing, is it template based or pxe based?

let me know if this repeats it self.
