Issue description:
"Login Module Extension Hierarchy" contains children of AbstractServerLoginModule. But there are missing two login modules; RealmDirect Login Module as child of UsernamePasswordLoginModule and also Remoting Login Module as child of AbstractServerLoginModule.

However, there exist also login modules which are not children of AbstractServerLoginModule: Kerberos, Client and RunAs Login Modules. 

Suggestions for improvement:
In "Login Module Extension Hierarchy" add reference to Kerberos, Client , RunAs, RealmDirect and Remoting Login Modules.
