Description of problem:

When adding a new Group in the web Admin UI, clicking on (Look Up LDAP Groups) no longer updates the form for the userid/password of the user to use for querying groups in LDAP.

Steps to Reproduce:
1. Add a new Group
2. Clicking on "Look Up LDAP Groups"
3. Form doesn't update with userid/password fields
