Description of problem:

I added the same provider - once by it's ip and once by it's hostname 

Version-Release number of selected component (if applicable):
cfme-5.5.0.13-1.el7cf.x86_64

How reproducible:
100%

Steps to Reproduce:
1. add a provider by it's ip
2. try to add the same provider by it's ip again (you'll get and error)
3. try to add the same provider by it's hostname

Actual results:

we are able to add the same provider once with it's ip and once with hostname

Expected results:

we should not be able to add the provider if it already exists regardless if we are using the ip or hostname
