Description of problem:
Go to "Admin" -> "Users" -> "Add LDAP User" and user cannot input anything to inputs. User cannot save this form or search. This page is not functional.

Steps to Reproduce:
1. Go to "Admin" -> "Users" -> "Add LDAP User"
2. try to input anything to any input
3. try to save form

Actual results:
"Admin" -> "Users" -> "Add LDAP User" is not functional

Expected results:
User can save this form with filled inputs and LDAP users can be mapped to USM. User can search users in  LDAP.

User cannot search untill ldap is configured. BZ 1310100 will make the 'add ldap user' tab disabled if ldap is not configured
