kernel-4.4.8-300.fc23.x86_64 does not have this problem, with otherwise identical configuration. I am unable to reproduce this now with either kernel-4.5.3-300.fc24.x86_64 or kernel-4.5.4-300.fc24.x86_64. Something's up because I'm having this problem intermittently also. Keyboard goes away, so I have to unplug plug then play again. But sometimes it affects bluetooth exactly 2 seconds after a USB reset happens, and this breaks bluetooth entirely for that boot once it happens, i.e. restarting bluetoothd doesn't work, and turning off bluetooth fails. I have to reboot.
