Description of problem:
With the pxe file distilled from RHVH-4.0-20160831.4-RHVH-x86_64-dvd1.iso, the boot program were failed at an early stage. 

Version-Release number of selected component (if applicable):
redhat-virtualization-host-4.0-20160831.4.x86_64

How reproducible:
100%

Steps to Reproduce:
1. Export pxe boot files from iso to pxe server
   a. mount -o loop ./RHVH-4.0-20160831.4-RHVH-x86_64-dvd1.iso iso /mnt/iso
   b. export /mount/iso/images/pxeboot/vmlinuz
   c./mount/iso/images/pxeboot/initrd.img via `cobbler distro add`
2. Ran into pxe boot
3. Boot with KS file defined below

Actual results:
1."System halted error" appear at an early stage before the anaconda

Expected results:
1. After step#3, anaconda appears and can install RHVH.

Additional info:
1. Since we can do pxe boot successfully with version RHVH-4.0-20160829.0-RHVH-x86_64 and other previous RHVH based on the same steps.
It's a regression on RHVH-4.0-20160831.4-RHVH-x86_64-dvd1.iso

2. Can not get logs since it happened at the early stage

It's a dracut error.

What is the kernel command line you use for booting?
