Verified on: CFME-5.6.2.1.20160922130607_92d5b5e RHV-4.0.4.2-0.1 VM is indeed provisioned & started on RHV side. Since the problem described in this bug report should be resolved in a recent advisory, it has been closed with a resolution of ERRATA.
