Description of problem:
If Multi-page HTML format is selected, the left menu doesn't show the first subchapter, e.g. 4.1, 5.1, etc.

Thanks, Lucie. This was caused by some kind of issue with tooling or templates, so I have logged a bug here:

I will close this BZ ticket because it isn't a docs issue per se.
