Description of problem:
There are 3 boot entries on multipath iSCSI server after installation, and after twice reboot, the 3 entries are still there. 
The 3 entries are as followed, the second entry "Red Hat Enterprise Linux..." should disappear after twice reboot.

This issue is same as Bug 1346872, but it only occurred on multipath iSCSI server now. For other servers, like multipath FC,singlepath iSCSI, the issue didn't occur.

Steps to Reproduce:
1. Install RHVH on a multipath iscsi server.
2. Reboot and focus on the boot entries.
3. Reboot again and focus on the boot entries.


Actual results:
1. After step2, there are 3 boot entries.
2. After step3, there are still 3 boot entries the same as step2.

Expected results:
1. After step2, there are 3 boot entries.
2. After step3, there are 2 boot entries, the "Red Hat Enterprise Linux..." entry should disappear.

FYI -- I'll probably request a test environment early next week if I can't set one up. Is this software or hardware iscsi?

The bug occurs on hardware iscsi.
Please needinfo qiyuan@ if you need QE provide the test env.

I don't have any systems with a hardware iscsi initiator.

Can you please provide an environement?

It has no functional implications, moving to 4.1.1
