v2v issues

Description of problem:
-Window not consistent with other windows
-Form fields misleading
-Terms not consistent

Version-Release number:
4.1

All the following issues are shown in the enclosed mock ups as well.

Expected results:
-General
--Remove “Name” from VM table
--Move “Load” to the right and add an <hr> to differentiate between the 2 parts.
--Change “Host” to “Proxy Host”, perhaps “Management HOST”

-Re-order “Source”  Drop down:
--KVM
--External...
--VMware
--VMware virtual appliance
--Xen

-Export Domain category reorder fields:
--Name
--Description
--Path
(Make all fields read only)

vCenter
-Reorder fields:
--Column 1:
---vCenter
---ESXi
---DC
---Cluster

--Column2:
---User name:
---Password:
---Verify server’s SSL certificate
(Enlarge proxy Host Select box width)


-KVM
--Move checkbox box before the word Require Auth
--Indent username and password (they should be placed under the word  “Require Auth”

-2nd page of the wizard
--Move “Clone” to the end of the grid
--Remove from the grid: "Origin" and "icon of VM Type" next to it.
--Attach virt Driver: move the checkbox before the text.
--VM ID should be shown as a field so it can take 1 line.
--VM Name can be renamed -> change field to enabled style.
--Icon of alert should be take from web_admin grid and not user_admin.
