Description of problem:
OpenStack cloud provider refresh fail with nil:NilClass when one the flavor ID has been changed or when the flavor has been removed while its still been reference by existing instance.

Error - 8 Minutes Ago undefined method `[]' for nil:NilClass.

Steps to Reproduce:
1. added OSP provider
2. after the provider refresh completed remove one of the existing flavor or change the flavor id ensure the flavor associate with instance
