cloud volume provisioning dialog

Description of problem:
Openstack Provisioning dialog allows users to add Cloud volumes.
Quota is not using the cloud volumes in its calculations


Steps to Reproduce:
1. Openstack VM provision, add cloud volumes with values(see attachment)

Actual results:
Cloud Volume values not added to quota requested values.

Expected results:
Cloud Volume values to be added to quota requested values.

Not seeing Quota using multiple volumes, because not able to actually add multiple volumes

Able to verify this on Version 5.9.0.12.20171205180333_3e32b3d
