Description of problem:

Fixes:

- no need to add osd to crush 
- remove HOSTNAME env requirement
- ceph.defaults: remove max_open_files
- ceph.defaults: drop /auth/cephx true and template
- config.static.sh: drop boilerplate
- start_mon.sh: only generate a monmap file when bootstrapping
- demo: fix mgr dahboard
- demo: fix nfs startup
- daemon: fix nfs log on stdout

Are these mapping to BZs? Please share the BZs and steps to verify these fixes.

There are no BZs. Nothing to verify, in particular, just make sure the container works.
