Do you have any idea what was going on at the time which might have triggered this?

this is very likely a dup of 144059, based on the partial backtrace in comment #1.

A fix for this problem has just been committed to the RHEL3 E5
patch pool this evening (in kernel version 2.4.21-27.0.3.EL).
