On the frontend the Cloudforms UI must allow the user to enter a custom endpoint in order to support Azure Stack.

The Azure provider code will then adjust the backend code to handle the endpoint.

The front end work should be nearly identical to the work that was already done for Amazon in https://github.com/ManageIQ/manageiq-ui-classic/pull/3958.
