Description of problem:
No cancel button is present on the policy simulation page of a infrastructure VM. 
Similar to BZ 1659088 but for infra VMs. 

Version-Release number of selected component (if applicable):
5.9.9.1

How reproducible:
100%

Steps to Reproduce:
1. Add an infra provider with 1 vm
2. Click the check box of the VM
3. Click Policy -> Policy Simulation

Actual results:
There is no cancel button when the page loads

Expected results:
There should be a cancel button there. 

Additional info:
Seems to be similar to BZ 1659088.

I check this again today, on 5.9.9.1 and 5.9.9.0 and am no longer seeing this issue. For now I am closing this as WORKSFORME, I will reopen if the issue manifests itself again. Note that BZ 1670456 still applies.

John,
the bug is still present, in the latest 5.9 and also in 5.10 so I am reopening the BZ.
Be careful about the steps to reproduce, I better add the exact steps:

1. Go to Compute > Infrastructure > Providers
2. Choose some provider, click on it to display its dashboard or summary page
3. Click on VMs in the page from step 2
4. Check the checkbox(es) of some VM(s)
5. In the toolbar, choose Policy > Policy Simulation
=> no Cancel button!

Important note:
If you access VMs directly thru Compute > Infra > VMs and follow the steps 2 - 5,
the bug will not occur.

Note that this bug is related not just to VMs, but any items (for example Templates)
displayed as a nested list thru some Relationships table.

Verified in 5.11.0.1.20190424210327_58b5dbd

Steps of verification:
1. Go to Compute > Infrastructure > Providers
2. Choose some provider, click on it to display its dashboard or summary page
3. Click on VMs in the page from step 2
4. Check the checkbox(es) of some VM(s)
5. In the toolbar, choose Policy > Policy Simulation
6. Verified that the cancel button is present and that it works when you click it
