Description of problem:
The volume seems be attached to the instance but did not exist in the SolidFire backend, making the instance useless. 
As a test, an instance was deleted with nova delete <instance>.  The theory was that it would delete it from “nova list” and it would delete it from “cinder list”, but it would also delete some other VM volume in Solidfire. This is exactly what happened: It deleted the volume associated with another instance.

This is fixed in 12.0.5+.

I would suggest retesting w/ a current openstack-cinder update.

NOTABUG,related to Solidfire which we don't have access to. 
Nothing to test/automate per close loop.
