I found a RFE reported in upstream but abandoned.

X-Forwarded-For and X-Forwarded-Port are HTTP headers for insertion available in OSP 13:

Just to be clear, are you referring to neutron-lbaas + HAProxy driver or Octavia + amphora driver?
