Description of problem:

I created a service dialog from an Ansible Tower template. The resulting service dialog didn't show a dropdown, like expected.

Steps to Reproduce:
-------------------
1.Add Tower. Wait for provider refresh to complete.
2.Navigate to Automation->Ansible Tower->Explorer. Click 'Templates'.
3.Select a Tower job template that uses a survey so that extra variables could be passed to Tower from CFME.The 'Answer Type' in the survey could be 'single select' or 'multiple select'.See attached screen shot of a Tower job template that uses survey.
4.On the Template details page, click 'Configuration', click 'Create service dialog from this template'.


Actual results:
---------------
The dropdown for the survey field is missing in the newly created service dialog. When I try to edit the dialog, the dropdown can be seen.

The service dialog should correctly show dropdowns .
