Description of problem:

When the RHCS 4.0 Installation Guide was released, the upgrade chapter only covered going from 3.x to 4.0, even though it said it covered both major and minor updates.

For 4.1 we need to update it so it covers minor updates as well (4.0 to 4.1)

One example of a step that needs to be updated is this:

Run the purge.yml playbook to remove an existing Ceph dashboard installation:

This is not required if going from 4.0 to 4.1, only from 3.x to 4.x. We need to add text that says to skip that step if going from 4.x to 4.x, or that the step is only for going from 3.x to 4.x
