Description of problem:

Our customers are facing this issue in the OCP 4.4.10

https://github.com/operator-framework/operator-lifecycle-manager/issues/1570 in OCP 4.4.

I couldn't find any BZ already filled for this and wasn't able to reproduce as well but our customers are able to reproduce this for multiple operators and the installplans never get removed, they persist.
