Description of problem:
Upload image need to approve the certificate, but after approve the certificate and back to form, it tells you that "the Operating system source already defined", this is very unexpected. It should be able to continue since it's just approve a certificate.

100%

Steps to Reproduce:
1. upload an image to golden OS
2. it pops up 'It seems that your browser does not trust the certificate of the upload proxy. Please approve this certificate and try again'
3. approve the certificate
4. it needs to back to form
5. "the Operating system source already defined" shows

Actual results:


Expected results:
It should be able to continue after approve the certificate.

verified on master, approve the certificate appears just after click upload, looks good.
