Several races have been reported connected with the allocation, referencing and
destruction of mapped devices which can lead to crashes.

This set of patches tightens up the code.

committed in stream U4 build 34.26. A test kernel with this patch is available
from http://people.redhat.com/~jbaron/rhel4/

Patch is in 40.1.EL, setting verified as there doesn't appear to be a test case
for this.
