Description of problem:
Try to create rhel6 VM from template, hit "Cannot read property 'value' of undefined" in the web page

Steps to Reproduce:
1. Login to web console, Project: openshift-cnv, click "Virtualization" -> Virtual Machines -> Create: Virtual Machine with Wizard,
Select a template: "Red Hat Enterprise Linux 6.0 + VM" -> Next,
   Boot source type: Import via URL (creates PVC)
   Import URL: http://s3-qe-cn-storage.usersys.redhat.com/libvirt-CI- 
   resources/RHEL-6.10-x86_64-latest.qcow2
   Persitent Volume Claim size: 10 GiB
Advanced: 
   Storage class: manual
   Access mode: Single User(RWO)
Click Next,
   Project: openshift-cnv
   Virtual Machine Name: rhel6
   Flavor: Small 1 CPU| 2 GiB Memory
   Storage: 2 Disks     Workload profile: server
   Boot source: Boot from disk
   http://s3-qe-cn-storage.usersys.redhat.com/libvirt-CI-resources/RHEL-6.10-x86_64-latest.qcow2
    Select: Start this virtual machine after creation

2. Click "Create virtual machine", hit error:
   "Error creating VM Cannot read property 'value' of undefined"

3.  Check all the items are with value.
4.  Click "Customize virtual machine" -> Networking: "Next" -> Storage: "Next" -> Advanced: "Next" -> Review: "Create Virtual Machine": Get message:
"Successfully created virtual machine."

Actual results:
In step2: Hit error: "Error creating VM Cannot read property 'value' of undefined", but nothing need input.


Expected results:
In step2: Create virtual machine successfully.
