Description of problem:

Install/Upgrade button on Install/Upgrade Helm Chart page does not work with Form View

Steps to Reproduce:
Following this (https://github.com/openshift/console/blob/master/frontend/packages/helm-plugin/integration-tests/features/helm/install-helm-chart.feature#L44) scenario to install a Helm Chart release using the Form View.

When "Install" button is clicked, nothing happens and in the Console the following error is shown (helm-install?chartName=nodejs-ex-k&chartRepoName=redhat-helm-repo&chartURL=https%3A%2F%2Fredhat-developer.github.io%2Fredhat-helm-charts%2Fcharts%2Fnodejs-ex-k-0.2.1.tgz&preselected-ns=default:1 An invalid form control with name='' is not focusable.

Need to switch to YAML view to be able to install it



Similarly, following this (https://github.com/openshift/console/blob/master/frontend/packages/helm-plugin/integration-tests/features/helm/actions-on-helm-release.feature#L29) scenario to upgrade installed chart:

"When ""Upgrade"" button is clicked, nothing happens and in the Console the following error is shown (upgrade:1 An invalid form control with name='' is not focusable.)
Need to switch to YAML view to be able to Upgrade"

 * OCP: 4.8.0-fc.3
 * Chrome: 90.0.4430.93
