Description of problem: Replace "Are you sure" text when confirming deletion of providers, mappings and plans

How reproducible: Everytime


Steps to Reproduce:
1. Attempt to remove a provider, migration plan, mapping, or hook
2. Or attempt to leave while in the middle of creating a plan
3. Notice the language prompted to the user

Actual results: Wording uses "Are you sure"


Expected results: Make title a question, and explain what will happen if user proceeds.
