Address pool name one marked required with * and one in bottom of form.

Description of problem:
There are to entry fields for address pool name, one field is marked required but other one is not.

How reproducible:
Always

Steps to Reproduce:
1. Install the metallb operator from web console in a project metallb-system from OperatoHub->Networking.
2. Create MetalLB instance from installed operator API.
3. Create Address pool from installed operator API.

Actual results:
Two entry fields from address pool name.

Expected results:
Should have only one entry field address pool name.
