Description of problem: The modal dialog about the support status of a VM created from a template contains a wrong annotation.

How reproducible:
100%

Steps to Reproduce:
1. Create a VM from a user-defined template which does contain the label template.kubevirt.io/provider, but not template.kubevirt.io/provider-url


Actual results:
Modal dialog displays:
"Template support
The support level is not defined in the template yaml. To mark this template as supported, add these two annotations in the template details:
template.kubevirt.io/provider: Your company name
template.kubevirt.io/provider-support-level: Full"

Expected results:
"Template support
The support level is not defined in the template yaml. To mark this template as supported, add these two annotations in the template details:
