the problem fixed on catalog review page.
but the problem still exist other places:
1. add new disk modal while creating the vm
2. edit existing disk modal while creating the vm
3. add new disk modal for a vm
4. edit existing disk modal for a vm

Verified by mistake, not all fixes are intact yet.
