certificate error is not dismissed

Description of problem:
When upload a disk image to PVC, don't approve the certificate error or cancel the upload, and then select another source like default/url, the error is not dismissed.

Steps to Reproduce:
1. create a vm in customize mode
2. select upload
3. don't approve the certificate error or cancel the upload

Actual results:
error is not dismissed after select other source

Expected results:
error is dismissed

the consequence is it cannot continue after select another source

the problem is not fixed completely, if you click the "review and create" button, the error still pops up.
