Add disk -> upload failed for template

Description of problem:
Add disk to a template and try upload, it failed with below error:

Error "Invalid value: "${NAME}-disk-frantic-chimpanzee": a lowercase RFC 1123 subdomain must consist of lower case alphanumeric characters, '-' or '.', and must start and end with an alphanumeric character (e.g. 'example.com', regex used for validation is '[a-z0-9]([-a-z0-9]*[a-z0-9])?(\.[a-z0-9]([-a-z0-9]*[a-z0-9])?)*')" for field "metadata.name".

Steps to Reproduce:
1. create or clone a template
2. go to disks tab and add disk
3. select source "Upload"

Actual results:
it failed to upload disk to the template

Expected results:
"Add disk -> Upload" works
