Cluster > Physical Disks > Identify form not displayed

Description of problem:

The Cluster > Physical Disks > Identify form is not being displayed in dashboard. Clicking the "Identify" Button just dims the screen.

How reproducible:

1/1


Steps to Reproduce:
1. Deploy a ceph 8.0 cluster
2. navigate to Cluster > Physical Disks and click on Identify
3.

Actual results:

A form must be displayed to select the duration of how long to blink the LED

Expected results:

No form is displayed and screen goes dim
