Cause: ceph-mgr daemon crashes due to using MgrOpRequest instance even though ref mem is already released.
Consequence: ceph-mgr daemon crashes during MgrOpRequest cleanup.
Fix: fix included a critical use-after-free bug
Result: ceph-mgr does not crash
