Hmm. This is due to how libvirt/xen handle hotplugging, and how virt-manager exposes it to the user. virt-manager should ensure that any hotplug operation is persistent across VM start/shutdown. This is fixed in upstream virt-manager, but it might be a pretty invasive/substancial backport.
