can you reproduce this reliably, or was it a one-time thing?

if you can reproduce it, can you get the pulseaudio -vvvv logs from reproducing it? run:

pulseaudio -k
pulseaudio -vvvv

in a console, that will run PA with verbose logging, then make it crash and paste the logs...
