It just seems to happen after a couple of hours use. I am running an xorg server with 2 monitors, with one of the monitors rotated right 270 degrees. It's frustrating as I lose all my state and work. I considered switching back to the nvidia driver, but that doesn't support xrandr from within kde very well and I'll have to come up with a custom xorg.conf which can take hours.
