Description of problem:
RHN Satellite 5.4.0 Reference guide contains lot of notes and/or
exceptions for Red Hat Enterprise Linux 2.1. Most of them can be
easily removed, to avoid user confusion.

Please review whether are these notes really necessary.

Removed all the instances I could find. Please raise a new bug if you spot any I missed.
