gdbsx, userspace part

Backported r20319.

Located in tools/debugger/gdbsx, compiled by default to avoid silent breakage, but not included in any package, because only devs are going to use it. (might be in a separate internal-only package in the future)

xen/include/public/domctl.h updated from hypervisor patch (Bug 678618)

Notable changes:
gdbsx makefiles were significantly modified.
Fixed to compile on ia64.
Added example to readme.

update domctl.h from hypervisor

Moved the tool from userspace to hypervisor repo.

The only thing done in userspace now is updating domctl.h with modified version from hypervisor.
