Patch for connection consumer leak in JCA/TCK testing.

In running the TCK with the new Qpid JCA adapter, a significant issue was found in correctly releasing consumers in the case of an app server shutdown. This affects the JCA adapter as well as Java messaging client.

Attached is a patch provided by Kevin Connor that should be applied to the current development code base to address this issue.
