This is a PDF that has text fill-in fields, something like what you download from IRS. When the file is opened from Acrobat.com everything is displayed correctly. When opened in Inkscape, the form is displayed, but the filled in text data is gone. At least the Inkscape does not crash.
However Evince silently crashes. Following is displayed in the terminal: 
Error (0): Call to Object where the object was type 4, not the expected type 6
Aborted (core dumped)

I've backported patch for poppler bug https://bugs.freedesktop.org/show_bug.cgi?id=33063 which fixes this problem.
