Description of problem:
pthread_mutex_init() and pthread_cond_init(). vdsm should not carry on happily in that case. Also, pthread_{mutex,cond}_destroy() must be called when the mutex/cond falls out of use.

Saw it once

Steps to Reproduce:
1. Probably use a lot of locks for a lot of time. no real idea.

Tested with 16 Vms. Each Vm has 3 HDs.
