There is a choose provider account button that doesn't do anything.

Steps to Reproduce:
1. Click administer
2. Click cloud providers
3. click accounts
4. click new account
5. then you are on the new provider account page


Actual results:

there is a "choose" button, right beside the "provider", and it doesn't do anything.
