Description of problem:
We are probably missing some validations for templates in changesets. It is possible to assign same template more than once.

Steps to Reproduce:
1. create a template and a changeset
2. run command for adding a template to the changeset twice
3. see changeset info
  
Actual results:
The template is listed twice.

Expected results:
It should raise an exception upon second addition.
